Booking your Surgery | How it Works
Our booking process is partially automated and personally handled by your patient facilitator, JoAnn Jackson, RN, BSN. Listed below is a short outline of the booking process to help you understand how we manage booking, scheduling, and facilitating payments for your surgery.
Step 1: Find out the Price
JoAnn Jackson, RN, BSN is an excellent resource with personal experience with both Lap-Band Surgery and Gastric Sleeve Surgery; having undergone both procedures with Dr Kuri. JoAnn often guides prospective patients dependent upon their personal health, goals, and objectives for weight loss surgery.
Once you have determined the surgery of your choice, the first step of the process is to fill out our online form to find out the price for surgery. Once filled out, you will receive a series of emails from Dr Kuri and our team, including an email with the price, services, and amenities included with your surgery type.
If you haven’t found out the price of your surgery, we invite you to fill out the form located directly to the right entitled “Find out the Price”. Make sure to select the drop down option to select your surgery of interest. Step 2: Schedule your Surgery Date
Your personal patient facilitator, JoAnn Jackson, RN, BSN will communicate with you directly on the phone or by email and coordinate with our surgery calendar to help you schedule your surgery date Once your surgery date is determined, we will add it to our system and change your status from a “prospective patent” to a “scheduled patient”. Once this is completed, you will receive a new series of emails requesting you to perform additional tasks to prepare for your surgery.
Step 3: Paying for your Deposit to Secure your Surgery Date
We require a partially refundable surgery deposit in the amount of $500.00 to secure your surgery date with Dr Kuri. This may be paid by a cashiers check, wire from your bank, or online payment. In the event you decide to cancel your surgery, $250.00 of your total deposit will be retained for clerical expenses and time allocated to assisting educating you on surgery options and booking your procedure.
You will receive an automated email with instructions on options on how to pay your deposit. All deposits for surgery are required to be collected at least 7 days prior to your surgery date. The fastest and easiest way to pay is online. However if you elect to pay otherwise, you will need to ensure you follow the instructions for sending manual payment by USPS mail to our PO Box.
Step 4: Fill out our Medical Questionnaire
Once your status has been changed to scheduled and your surgery date has been set, you will receive an automated email with information and a link requesting you to complete our online Medical Questionnaire. All of your personal medical information is kept completely confidential and secure. None of your information is every shared with any third parties and is strictly obtained for purposes of evaluating your personal medical information to prepare you for surgery.
Step 5: Follow our guide entitled “Essential Information for Future Patients”
You will receive an email entitled “Essential Information for Future Patients” which includes information on preparing for surgery by following our guide on nutrition, exercise, support from family and friends, having a checkup or physical by your doctor, and suggested reading materials.
Depending upon your personal health, BMI, and other conditions; Dr Kuri may require you be on our “Liver Shrinking Diet” for a specific period prior to surgery. The liver is a very large organ in the obese, and it can easily bleed during surgery. We have found that the special LSD (Liver Shrinking Diet) helps to shrink and firm the liver, so it presents much less surgical risk. A smaller liver means a shorter and safer surgery. Please follow the diet very carefully. Your patient facilitator, JoAnn Jackson RN, BSN will provide you with this information and requirements if needed.

Step 6: Prepare to Travel for Surgery
Once you have completed all the above tasks, you will receive an email with information to help assist you with your travel plans. Some of these topics include methods of travel (Flying or Driving), Travel Document Requirements, Bringing a Companion, Accomodations and Transportation in Mexico, and other important contact information.
Step 7: Pay the Balance of your Surgery
You will receive an automated email 7 days prior to your surgery to pay the balance of your surgery. You will be given two options to pay for the balance of your surgery. You may pay directly online or you may elect to Pay by Cashiers Check, Money Order, or Travels Check the day before your surgery. No personal checks will be accepted.
If you wish to pay the balance of your surgery prior to the 7 days before your surgery, please contact your Patient Facilitator, JoAnn Jackson, RN, BSN and she will manually send you the email link to pay your balance online.
