In order to secure your surgery date, we require a deposit of $500.00. You have two convenient payment options available:

1. Pay Online: Simply use your credit card or PayPal account to make a secure payment by clicking the “PAY DEPOSIT ONLINE” button. You will be directed to a secure webpage to complete your transaction.
2. Pay by Mail: If you prefer, you can choose the “PAY BY MAIL” option. This will lead you to a page with clear instructions on how to print and send your deposit using a personal check or cashier’s check.

All Surgery deposits are facilitated by our patient facilitator JoAnn Jackson, RN, BSN.   If you wish to book surgery with Dr Kuri, please fill out a form on our website or contact JoAnn directly and she will help you begin the process.

Important Note: We do offer a partial refund for your deposit. If you decide to cancel without rescheduling, please be aware that $250 will be retained to cover the expenses associated with our materials, time, and the expertise in bariatric surgery.
